Summit Medical Carolinas provides chronic care management for seniors living with long-term conditions such as diabetes, heart disease, hypertension, and COPD in the Charlotte, North Carolina area. Our team monitors patients between visits, coordinates medications and specialists, and creates personalized care plans to reduce complications and hospitalizations.
Our Transitional Care Management and Chronic Care Management programs provide additional support during these shifts in care, and are set up to provide opportunities.
